The Field Nurse Practitioner is responsible for managing patient care and treatment (in person, telephonically and/or video).How will you make an impact & Requirements
How you will make an impact:
Provides a range of primary, acute and/or specialty healthcare services for patients in clinics, virtually or in home settings.
In states that do not require collaborative agreements, functions as the primary care provider.
Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices.
May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team.
Formulates and documents care plan. Prescribes medication or other forms of treatment.
Participates in On-Call program, which may include after-hours, weekend and holiday calls and visitations.
Minimum Requirements:
Requires an MS in Nursing and minimum of 1 year of nursing experience; or any combination of education and experience, which would provide an equivalent background.
Requires a current, active, valid and unrestricted RN license and NP license in California.
For Carelon Health, satisfactory completion of a Tuberculosis test is a requirement for this position.
For candidates working in person or remotely in the below location(s), the salary* range for this specific position is $ 121,440 to $ 182,160.
