Clinical Specialist 
 
An experienced Advanced Practice Provider, who sets a standard of excellence from a national support perspective. This is a dynamic role that has two distinct parts, one in supporting excellent clinical care through chart reviews and the second in developing and presenting education, clinical programs and training for the providers and ancillary staff at apree health. Apree’s mission and clinical model support practitioners who focus on patient care first, develop the patient- provider relationship, and use empathetic listening to empower patients to actively participate in their own wellness goals. As an experienced healthcare leader and provider, you will partner with other health professionals to assure high quality acute, chronic, and preventive care needs are well documented and delivered. You will work alongside and collaborate with a knowledgeable and independent professional and allied staff to enhance the delivery of exceptional patient care.
 
Position Qualifications/Essential Functions: 
  • Adheres to all requirements of the provider role.
  • Working with the clinical leadership team to assure development and delivery
    of excellent evidenced-based clinical practice through company-wide mentoring and consultation, orientation, training, and performance management programs to build employee engagement and sustain motivation to deliver exceptional whole patient care.
  • Assesses the clinical and operational needs of the providers and support staff and provides recommendations relating to clinical services to enhance and maintain our high quality and exceptional patient experience This is done through trending of quality, operational, clinical and patient experience metrics and interaction with the field leadership and field support teams.
  • Review charts for accuracy, consistency, and evidence-based guidelines, documentation to the highest level of specificity, closure of care gaps as appropriate.
  • Participates in the development and presentation of ongoing clinical and operational trainings to team members through innovative teaching tools while using adult learning theory.
  • Ongoing review of clinical documentation for providers and development of teaching plans to address trends and gaps using USPSTF guidelines and evidence-based practice.
  • Completes chart reviews to ensure evidence-based medical care is being documented and completed in the clinics and escalates concerns to leadership with plan for addressing concerns with providers. This will include doing chart reviews on all new providers and those requiring remediation.
  • Partners with providers in an esteeming way to review charts and documentation.
  • Assesses the educational needs of the providers through chart reviews and open and respectful discussions with the providers and develops and delivers training and education to the teams based upon USPSTF guidelines and evidence-based practice.
  • Familiarity with a Medicare population including relevant quality metrics, HCC/Risk Adjustment diagnosing and coding of chronic conditions Stars and HEDIS Measures. Collaborates with Quality Improvement Committee to manage the clinic Quality Improvement program.
  • Works with clinical teams to achieve target quality objectives to meet company established benchmarks for clinical outcomes, engagement & patient satisfaction.
​Additional Qualifications: 
  • Minimum 2 years direct patient care as an advanced practice provider, with case management experience, emphasizing wellness and health promotion and population health management.
  • Minimum 2 years computer skills, including EMR experience required
  • Prefer experience in curriculum development for a clinical audience
  • Minimum required Master of Science in Nursing (MSN)
  • Current nurse practitioner licensure (valid in one of the states we are operating in, in good standing).

Compensation:  $115k-$138k/annual salary (national average, premium markets may vary)
